Q: "I’ve lost my Identification Card-how can I get a new one?"

A: A member can call the insurance carrier directly or may call our office for assistance in obtaining a new ID card. Some insurance carriers now have areas of their websites as well that allow you to request one online-we can also show you how to do this.

Q: "How can I request another Certificate of Coverage book ?"

A: A member can call our office anytime & we would be happy to answer any benefits questions they may have. If they would like to find coverage information online or order a benefits booklet, some insurance carriers now have these available to order or view online. Our office can assist them with where to find this information.

Q: "What is an EOB?"

A: The letters "EOB" stand for "Explanation of Benefits". This is mailed to members after a claim has been received, to show what action has been taken on their claim, & what the member owes and/or insurance company paid. These are important documents that should be retained by the member.

Q: "My Explanation of Benefits has codes listed after some of the charges-what does this mean?"

A: When an EOB has codes listed & no payment was made by the carrier, see the "Remarks Section" of the EOB to find what action was taken on the claim. There are various reason’s listed as to why payment may not have been made on the claim that would be listed there. If it is not clear or you need assistance, please contact our office.

Q: "I received a bill from my doctor-how do I know if what it shows I owe is correct?"

A: Members should match bills they receive with the insurance EOB by the date of service and amount of the claim. If the bill & EOB do not match, the member may contact our office for claims assistance.

Q: "My doctor’s name isn’t in the provider directory-how can I find out if they are in-network?"

A: The book directories are subject to change. The most accurate & current provider listings are now usually listed on an insurance carrier’s website. If you need help in locating the website or navigating, please contact our office & we can help you to work with the website or find a provider for you if you do not have Internet access.
